2. Valid Photo ID - three copies

3. Birth Certificate - three copies

4. Original Dual Citizenship documents (Oath of Allegiance, Order of Approval and Identification Certificate) original set and three copies

5. Original Philippine Passport (if available) - one original and three copies

6. Self-addressed pre-paid Express Post envelope with tracking number
