BASIC REQUIREMENTS NEWBORN/MINOR NEW
e-PASSPORT APPLICATION

​​Core Requirements

  1. Confirmed Appointment (Click for DFA Global Appointment System [GOAS])

  2. Accomplished Application Form 

  3. Personal Appearance of minor applicant and either parent or authorized adult companion

  4. Philippine Statistics Authority (PSA) Birth Certificate or Report of Birth
    - If born in the Philippines: Original and photocopy of Birth Certificate issued y the Local Civil Registrar if PSA - issued Birth certificate is not clear or cannot be read.
    - If born abroad: May submit original and photocopy of Report of Birth issued by the Philippine Consulate General in Sydney within one (1) year from the date of registration.

  5. Marriage Certificate is required if only one parent is accompanying the child
    - If parents are unmarried, a Special Power of Attorney (SPA) executed by the Mother will be required if she is not accompanying the child

  6. Original and photocopy of parent's passport

  7. School ID (if applicable)

  8. Photocopy of Australian Visa (if applicable)

  9. A non-refundable passport processing fee to be paid in cash or postal money order payable to the “Philippine Consulate General”. (Consular Fees)

  10. AUS Post (domestic - plastic) envelope with signature required upon delivery, to be used in mailing out of your new passport once it is received from the Philippines.  Please click here for a sample.

 

Supporting Documents for Minor New Applications

In addition to the core requirements, the following supporting documents will be further required in the following cases:

In case applicant is a newborn (less than 1 year old) and has No PSA Birth Certificate/Report of Birth yet
• If born in the Philippines - The minor applicant may submit a Certified True Copy
(CTC) of Local Civil Registrar (LCR) Birth Certificate authenticated by the PSA.
• If born abroad -The minor applicant may submit original copy of Report of Birth (ROB) issued by the Philippine Consulate General in Sydney within one (1) year from the date of registration.


In case the minor applicant's PSA-issued Certificate of Live Birth was registered one (1) year after the event (delayed registration of birth), the applicant must submit at least one (1) of the following supporting documents:

  1. School Records; or

  2. Baby book or health record or its equivalent document.

In case applicant is not accompanied by the parent/s during the application process

  1. Applicant must submit a Special Power of Attorney executed by the parent/s designating the minor’s companion to assist in the application process.

 

In case applicant is not traveling with parent/s

  1. Applicant must submit Waiver of Exclusion Ground (WEG) clearance and Affidavit of Support and Consent executed by the parent/s. The Affidavit of Support and Consent must be Consularized by the Philippine Embassy/Consulate. (download forms under notarial)

 

In case applicant is an illegitimate child in custody of the Mother

  1. Personal Appearance of minor applicant and mother or authorized adult companion

  2. PSA Birth Certificate

  3. Passport of mother or authorized Adult Companion 

  4. If applicant is not accompanied by mother during the application process, applicant must submit a Special Power of Attorney executed by the mother designating the minor’s companion to assist in the application process.

  5. If applicant is not traveling with Mother, applicant must submit Waiver of Exclusion Ground (WEG) and Affidavit of Support and Consent executed by the mother.  The Affidavit of Support and Consent must be authenticated by the Philippine Embassy/Consulate. (download forms under - notarial)
