No ability to go to the Consulate to get the NBI fingerprint Form 5 because you are far away?

You can request the Consulate to send you the NBI fingerprint Form 5, just follow these steps:

  1. Make a letter request or a letter asking for NBI Form 5 from the Consulate

  2. Indicate in the letter if you are Filipino or a foreigner
    Include Proof of nationality (copy of the data page of your VALID passport, either Philippine or foreign passport)

  3. Send this to the address: 
    Philippine Consulate in Sydney Philippine Center,
    Level 1, 27-33 Wentworth Avenue
    Sydney, NSW 2000 Australia

  4. Don't forget to include an extra AUSPOST self-addressed stamped envelope inside the envelope (This will be used to send you the NBI fingerprint Form 5)

  5. Once we receive your request, we will send you a blank NBI fingerprint Form 5 using the extra envelope you provided.

  6. Go to the nearest police station and bring NBI Form 5. (better if you bring your own stamp/ink pad)

  7. Have your fingerprint taken in front of a Police Officer (they should not forget to initial the form as proof that the fingerprinting was done in front of them).

  8. Return/send the accomplished NBI Form 5 to the Consulate address along with the following:

  • Processing fee of 45$AUD in postal money order

  • Proof of nationality (copy of the data page of your VALID passport, either Philippine or foreign passport)

  • Extra self-addressed stamped envelope. This will be used to send back to you the NBI fingerprint Form 5 signed by our Official (Consul or Vice Consul)

  • After you received the accomplished/signed NBI fingerprint Form 5, then follow this steps:


         Through an Authorized Representative

         Requirements:

    • Photocopy of previous NBI Clearance

    • Valid passport two (2) photocopies of data page (page showing photo and complete name) INCLUDING page with signature

    • ID photoone (1) colored 2”x 2” photo with white background, taken within the last six (6) months, in proper attire. Blurred, scanned, or low-quality photos will not be accepted

    • Signed Authorization Letter (wet signature) from the applicant for the representative (As advised by the NBI Main Office, an authorization letter bearing the applicant’s original wet signature will suffice; a Special Power of Attorney is not required.)

    • Valid ID of the representative – Photocopy

      Processing and Next Steps

    • Fingerprinting at the Consulate is not required. Applicants may send the requirements to their authorized representative for onward submission to the NBI Main Office in Ermita, Manila.

    • Upon receipt of the clearance certificate, the applicant must examine it for the presence of embossed NBI dry-seal and then affix their right thumbmark on the space provided.



      Through Mail (No authorized representative)

      Requirements:

    • Original NBI Clearance (expired, for renewal) or the NBI Fingerprint Form 5

    • Valid passportone (1) photocopy of data page (page showing photo and complete name) INCLUDING page with signature

    • ID photo – one (1) colored 2”x 2” photos with white background, taken within the last six (6) months, in proper attire. Blurred, scanned, or low-quality photos will not be accepted

    • Payment of 200 PHP or its equivalent in US dollars

    • Self-addressed, self-stamped envelope from AusPost or any courier WITH TRACKING 

      Processing and Next Steps

    • If the applicant does not have an authorized representative to process the application on their behalf, they may send their application directly to:

    • Processing by NBI Manila takes a maximum of five (5 working days) upon receipt of documents. After processing, the NBI sends the clearance certificate to the applicant in Australia.

    • Upon receipt of the clearance certificate, the applicant must examine it for the presence of embossed NBI dry-seal and then affix their right thumbmark on the space provided.

      Other notes:

    • If there are changes to the details of the NBI Clearance such as civil status, name, date of birth, the NEW APPLICATION applies.

    • If the applicant’s address in Australia has changed and needs to be updated, a letter indicating the new residential address must be included in the requirements.
