AFFIDAVITS AND NOTARIAL SERVICES

Note:

  • NOTARIAL SERVICES ARE BY APPOINTMENT ONLY. Please read the requirements before booking an appointment

  • NO APPOINTMENT NEEDED for Senior Citizens, Persons with Disabilities, and Pregnant Women 

  • All parties signing the documents must appear in person.  However, the Attorney in Fact for powers of attorney is not required to appear before the consular officials, and may sign after the receipt of the document.

  • Witnesses are not required to appear before the consular officials.

REQUIREMENTS FOR NOTARIZATION​
 

ACKNOWLEDGEMENT ​
Documents such as Special Powers of Attorney, General Powers of Attorney, Deeds of Sale, Deeds of Donation, Contracts, SSS/GSIS forms, Extrajudicial Settlement, etc.:​
 

  • Personal appearance of the signatory/ies;

  • Original and photocopy/ies of the unsigned document including all attachment/s. The document will be signed in front of the Consular Officer.

  • Proof of Identity: The person(s) executing the document must present two (2) original valid identification documents (e.g., Passport, State ID, Driver’s License). A copy must be provided for each original document submitted.  For the photocopy of the valid ID, please take note of the following:

    • The copy must be clear and legible, showing the full name and signature of the principal(s).

    • The valid ID presented must contain a signature and reflect the complete and correct name of the principal, including Given Name, Middle Name, and Last Name.

    • Please note: The name that will appear on the Consulate’s Acknowledgment page will be based on the name as shown on the valid ID, not the name as written in the document to be signed.


JURAT
Documents such as Affidavit of Support and Consent, Parental Travel Permits, Complaints, Applications for Marriage License and Oaths of Professionals:

 

  • Personal appearance of the signatory/ies;

  • Original and photocopy/ies of the unsigned document including all attachment/s. The document will be signed in front of the Consular Officer.

  • Proof of Identity: The person(s) executing the document must present one (1) original valid identification document (e.g., Passport, State ID, Driver’s License). A copy must be provided for each original document submitted.  For the photocopy of the valid ID, please take note of the following:

    • The copy must be clear and legible, showing the full name and signature of the principal(s).

    • The valid ID presented must contain a signature and reflect the complete and correct name of the principal, including Given Name, Middle Name, and Last Name.

PROCESSING AND RELEASE:

  • Schedule of fees (to be paid by applicant in cash or via debit/credit cards, Australian dollars only)

    • Affidavit – AUD 45

    • Expedite fee for same-day processing – AUD 18

       

 

Regular Processing

  • Pick-up at 1:00 PM the following business day upon receipt of complete requirements.

 

  • By mail sent via AusPost before end of business hours the following day [Applicant to avail of self-addressed Express Post (plastic) Envelope with tracking number.] The Consulate assumes no responsibility for any delay, loss or damage in the mail or while the documents are in custody of the courier service. The applicant should take note of the tracking numbers of all envelopes used and submitted.
     

Overtime (EXPEDITE) processing

  • Only documents submitted before 12:00 NN may be accepted for expedite processing, except under meritorious circumstances. These are released by 3:00 PM on the same business day, upon receipt of complete requirements. An expedite fee of A$18.00 will apply.
