AFFIDAVITS AND NOTARIAL SERVICES
Note:

All services are by appointment only. Please read the requirements before booking an appointment

All parties signing the documents must appear in person. However, the Attorney in Fact (AIF) for powers of attorney is not required to appear before the consular officials, and may sign after the receipt of the document.
REQUIREMENTS FOR NOTARIZATION
ACKNOWLEDGEMENT
Documents such as Special Powers of Attorney, General Powers of Attorney, Deeds of Sale, Deeds of Donation, Contracts, SSS/GSIS forms, Extrajudicial Settlement, etc.

Personal appearance of the signatory/ies;

Original and photocopy/ies of the unsigned document. Document will be signed in front of the Consular Officer;

Two (2) original government issued IDs with photocopies (such as, passport, citizenship card or driver's license), for each document to be notarized/acknowledged/consularized. The government ID should be valid and should support the claim of citizenship on the document. Dual citizens are advised to bring their Philippine passport or Philippine government issued ID;

Processing fee of AU$45 (Cash/Card or Postal Money Order only) per document, payable to the Philippine Consulate General; and
-
Self-addressed Express Post (plastic) Envelope for regular processing and release by mail the following working day.
JURAT
Documents such as Affidavit of Support and Consent, Parental Travel Permits, Complaints, Applications for Marriage License and Oaths of Professionals:

Personal appearance of the signatory/ies;

Original and one (1) photocopy of the unsigned document. Document will be signed in front of the Consular Officer;

One (1) original, valid government issued IDs with photocopies (such as, passport, citizenship card or driver’s license), for each document to be notarized/acknowledged/consularized. The government ID should be valid and should support the claim of citizenship on the document. Dual citizens are advised to bring their Philippine passport or Philippine government issued ID;

Processing fee of AU$45 (Cash/Card or Postal Money Order only) per document, payable to the Philippine Consulate General; and
-
Self-addressed Express Post (plastic) Envelope for regular processing and release by mail the following working day.
RELEASING TIME: -
Document will be released by mail the next business day from receipt of complete requirements.

The Consulate assumes no responsibility for any delay, loss or damage in the mail or while the documents are in custody of courier service. The applicant should take note of the tracking numbers of all envelopes used and submitted.
