All Filipinos are required to report important birth of their children, change of civil status such as marriage, which the Embassy/Consulate forwards to the Philippine Statistical Authority (formerly NSO).​ An appointment is required for personal submission. Requirements may also be sent by mail.​ The Consulate receives payment in cash in AUD, debit, credit card, EFTPOS, bank cheque, card or postal money order payable to the “Philippine Consulate  General”.

NOTE:  Incomplete documents will be returned. Do not staple. Kindly organize them in four (4) sets instead.

REPORT OF BIRTH

The birth of a child born in Australia of Filipino parents should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Consulate General.

Processing and Release Time: 

By appointment: 15 minutes (processing) 

By mail: The document will be mailed the next day after receipt of complete requirements      

Requirements:

  1. Duly accomplished Report of Birth Fillable Form* (5 originals). The form must be typewritten. Kindly ensure that all information is accurate to prevent the delay in the transmittal to (PSA). If submitting by mail, the form shall be notarized.

   2. Original and 5 photocopies of the following:

  • Australian Birth Certificate issued by the Registrar of Births, Deaths and Marriages 

  • Photocopy of Philippine Passports of Parents

  • Parent/s proof of status, i.e., Visa issued by Australia, Dual Citizenship documents 

  • Marriage Certificate of Parents issued by the Philippine Statistics Authority (PSA) (if married) or Report of Marriage of Parents (if married overseas)

*if application will be submitted by mail, Do not send the original documents in the mail. Documents must be certified true copy by the Justice of the Peace or Notary Public if the application will be mailed.

3. Processing Fee of AUD$45 in cash/card or postal money order payable to the Philippine Consulate General. Do not send cash in the mail.

4. Self-addressed Express Post (plastic) Envelope with contact details (address / email / mobile number).

5. Contact details (mobile/landline number) should be indicated on the self-addressed stamped envelope. 

Additional Requirements:

​If Parents are not married and the child will use the surname of the Father, the following are required:

  1. Father shall execute an Affidavit of Admission of Paternity (AAP)
  2. Mother/Guardian shall execute an Affidavit to Use the Surname of the Father (AUSF)

    1. If child is 0-6 years old; AUSF to be executed by mother

    2. If child is 7-17 years old; AUSF to be signed by the child with Sworn Attestation of Mother/Guardian

    3. If child is 18 years old above; AUSF to be executed by child with no required Sworn Attestation

  • If Parents were married only after the child’s birth, submit an Joint Affidavit of Legitimation  with PSA issued Birth Certificate of Mother.​

  • Submit a Affidavit of Delayed Registration of Birth (for Delayed Reporting 1 year after the date of birth).

  • If reporting 5 years after the date of birth, a recently-issued original PSA certificate of no report of birth record is required.

*Please note that each affidavit has an additional fee of AUD$45.00

 

The Consular Officer reserves the right to require additional proof of documents from an applicant to prove his/her citizenship, status or identity pursuant to the Philippine Passport law (RA 8239) and the Foreign Service Act (RA 7157).

 

Releasing

  • The personal copy of the ROB will be sent by mail the next 3-5 working days from receipt of complete documents 

  • The Consulate assumes no responsibility for any delay, loss or damage in the mail or while the documents are in custody in courier service. The applicant should note the tracking numbers of all envelopes used and submitted.

  • If the applicant will also apply for a Philippine passport, please bring additional copy of the ROB application and complete requirements during your passport appointment.

     

HOW TO GET A PSA COPY OF THE REPORT OF BIRTH

The Philippine Consulate General will transmit the ROB to the Philippine Statistics Authority, through the Department of Foreign Affairs. There is a waiting period of at least 4 to 6 months for the availability of PSA copy in security paper. Please get it from PSA by providing the transmittal details (Despatch number, Reference Number and Date of transmittal).

The PSA copy will initially be available at PSA, Solid Bldg., Magsaysay Blvd., Sta. Mesa, Manila.
