BASIC REQUIREMENTS ADULT RENEWAL (e-PASSPORT) APPLICATION​

Core Requirements

  1. Confirmed Appointment (Click for DFA Global Appointment System [GOAS])

  2. Accomplished Application Form 

  3. Personal Appearance

  4. Current ePassport with photocopy of data page

  5. Original and one (1) photocopy of PSA authenticated documents that will support the change of name, if applicable.
    - Marriage Certificate
    - Annotated Birth Certificate
    - Annotated Marriage Certificate to show annulment/divorce/court ordered instruction
    - Death Certificate of Spouse

  6. Photocopy of Australian Visa (if applicable)

  7. A non-refundable passport processing fee to be paid in cash or postal money order payable to the “Philippine Consulate General” (Consular Fees)

  8. AUS Post (domestic) pre-paid (plastic) envelope with signature required upon delivery, to be used in mailing out of your new passport once it is received from the Philippines. (sample)

 

NOTE: If woman opts to retain maiden name, a Marriage Contract is required. Additional documents may be required.

Supporting Documents for Adult Renewal of ePassport Applications
Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant's identity, citizenship and lack of travel restrictions, as specified in the following cases:

If a female applicant (married abroad) who wants to change her name from single to married name in her passport application must submit original copy of Report of Marriage (ROM) issued by the Philippine Consular in Sydney within one (1) year from the date of registration.

Reversion to Maiden Name (Renewal Applicant)
For applicants who wish to revert to their maiden names, may do so under the following modes provided below:
By Virtue of Death of Spouse:
1. Submission of an original and photocopy of a PSA-issued Certificate of Death (COD) or Report of Death (ROD) of spouse, or apostilled or authenticated Foreign Death Certificate with English translation, if applicable;
2. An original and photocopy of a PSA-issued Certificate of Live Birth or PSA Report of Birth;
3. Latest-issued Philippine passport (if available)


By Virtue of Annulment, Declaration of Nullity of Marriage, Judicially-recognized Foreign Divorce, Judicially-recognized      divorce under PD No. 1083, and Legal Separation:

  • Submission of an original and photocopy of a PSA-issued Certificate of Marriage  (COM) or PSA Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage, judicial recognition of foreign divorce, court decree of legal separation;

  • An original and photocopy of a PSA-issued Certificate of Live Birth or PSA Report of Birth;

  • Latest-issued Philippine passport (if available).


For those who wish to revert to their maiden names:

  • An original and photocopy of a PSA-issued Certificate of Live Birth or PSA Report of Birth;

  • An original and photocopy of a PSA Certificate of Marriage or PSA Report of Marriage

  • Notarized Affidavit of Explanation that includes a request for the reversion of maiden name in the Philippine passport or travel document and stating that the applicant has not hitherto availed of the reversion; and

  • Latest-issued Philippine passport or travel document; and

  • Any valid and existing Government-issued ID accepted for passport application, reflecting the maiden name of the   passport applicant.


The New Passport Law and its Implementing Rules and Regulations stipulate that a married woman who has used her husband’s surname in her travel document can revert to her maiden name only once (upon severance of every subsisting marriage)

 

Supporting Documents for Adult Renewal of ePassport Applications

In addition to the core requirements, the following supporting documents will be further required for the following cases:

 

NOTE: Some cases will require the application to be treated as a New Application rather than a renewal.

 

In case of a Lost VALID ePassport – will be treated as NEW APPLICATION

  • If photocopy of the applicant's last issued passport is available, the Birth Certificate will not be required.

       Additional Requirements for Lost Valid ePassport

In case of a Lost EXPIRED ePassport – will be treated as NEW APPLICATION

  1. Confirmed appointment (Click for DFA Global Appointment System [GOAS])

  2. Police Report in English (Original and photocopy)

      If photocopy of the applicant's last issued passport is available, the Birth Certificate will not be required.

 

In case of renewal of a frequently Lost Passport ;

  1. Police Report in English;

  2. Notarized Affidavit of Loss in English


In case passport has been mutilated or damaged

  1. Notarized Affidavit of Explanation


Note: On top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.


In case applicant is a Dual Citizen
Natural-born Dual Citizens:
1. Latest-issued Philippine passport; and
2. PSA-issued Certificate of Live Birth or PSA-issued Report of Birth.

Dual Citizens who availed of the provisions of Republic Act No. 9225:
1. Latest-issued Philippine passport;
2. Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine FSP; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by BI; and
3. Valid government-issued ID or their secondary citizenship/residence counterparts.

In case Applicant is a Naturalized Citizen
1. Latest-issued Philippine passport; and
2. Identification Certificate of Naturalization from the Bureau of Immigration, or Certificate of Naturalization issued by the Special Committee on Naturalization (SCN), or copy of the final Court Judgment or Law granting Philippine citizenship.

In case applicant is a Filipino citizen by Election

  1. Latest-issued Philippine passport;

  2. Documents of election of Philippine citizenship; and

  3. Identification Certificate issued by the Bl.
