I Lost my ePassport, what will I do?
 

In case of a Lost VALID ePassport – will be treated as NEW APPLICATION

  1. Confirmed Appointment (Click for DFA Global Appointment System [GOAS])

  2. Click here for Requirements for New Adult Applicants

  • If photocopy of the applicant's last issued passport is available, the Birth Certificate will not be required.

    Additional Requirements for Lost Valid ePassport

In case of a Lost EXPIRED ePassport – will be treated as NEW APPLICATION

  1. Confirmed appointment (Click for DFA Global Appointment System [GOAS])

  2. Police Report in English (Original and photocopy)

  3. Click here for Requirements for Adult New Applicants
     

If photocopy of the applicant's last issued passport is available, the Birth Certificate will not be required.

 

In case of renewal of a frequently Lost Passport ;·

  1. Police Report in English;

  2. Notarized Affidavit of Loss in English


Note: On top of the requirements above, the PSA-issued Certificate of Live Birth will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.

Please note that one of the requirements is the printed copy of the police report, not just a reference number from the police   station.  We cannot process your application without the printed copy of the police report.

I Lost my NON ePassport, what will I do?

This is an additional requirements on your application

 

In case of Lost expired NON-ePassport

  1. Police Report in English;

  2. Notarized Affidavit of Loss in English; and

In case of renewal of a frequently Lost Passport

  1. Police Report in English;

  2. Notarized Affidavit of Loss in English; and


    Please note that one of the requirements is the printed copy of the police report, not just a reference number from the police station.  We cannot process your application without the printed copy of the police report.
